Note that I am not suggesting any of the below.
They are just observations.

Some places have taken to reversing the genders of the
common (white) cable. I don't remember, but they might be
reversing the ground connectors as well. That makes those
connections look different from  the "hot leads.

Back in the old days of trico connectors sometimes the
common was a different size connector (larger).

At the source some boxes have mechanical interlocks that
fource you to make the camlok connections in the correct order,
but that doesn't help if yo are using the wrong connection.

The best deal is when you can use a connector where all of the
connections are on the same connector. Unfortunately the cables
get too heavy when you get much beyond 60 amps.
